Ran this on my Niner One9 over 14 years ago. Frame cracked in 2010 and hasn't been used since. Includes five pieces - top and bottom bearings, top race and top cap. I'm including the fork race but unfortunately it looks like I used a screwdriver to remove it, sorry. It may be still usable but also may need to buy a new one.
